Below please find the information we need from you, our patient, to get started.
We offer all our patients a meet and greet telephone call to determine the best way to proceed with treatment and to better understand the goals of the patients. Prior to this telephone call, we encourage each patient or his/her referring physician to send us a one-page summary (by mail, fax, or email). Once we agree to move forward with treatment, we will need you to fill out the appropriate forms. Please complete and transmit the forms to us (use our links in the Patient Information section). In addition, we will need all medical records including pathology, radiology, and treatment protocols since diagnosis sent to us. Again, this may be done by mail, fax, or email. Lastly, we also need a credit card number for payment.
Once Dr. Ripoll reviews the case, she will conduct a detailed consult on each patients unique case. Reviewing the information can take up to an hour. Then, we will telephone you to set up a consult with Dr. Ripoll, who will go over the entire treatment protocol. The fee for this consultation work is $300 per hour and the average consultation averages 2-3 hrs.
